Details of Experience:
Ms. Eleanor George arrived at Sans Souci Rehabilitation and Nursing Center on August 29, 2019, from St. John’s Riverside Hospital. Prior to coming to Sans Souci, Ms. George suffered a fall that led to a fracture on the right side of her pelvis. As a result, Ms. George was unable to walk and required a wheelchair for ambulation. Upon completion of her hospital stay, Ms. George was referred to Sans Souci for physical rehabilitation to regain independence walking, navigating stairs, and performing activities of daily living (ADL’S).

Upon admission, Ms. George was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Ms. George to our community and conducted evaluations to custom tailor her stay.

Ms. George was evaluated by members of the rehabilitation team, Dexter Galette COTA, and Sam Beloni DPT, who performed an initial evaluation for her. Ms. George’s rehabilitation goals were to safely perform bed mobility tasks and functional transfers independently, ambulate 250 feet with a rolling walker, and safely ascend and descend 8 steps on the stairs. Ms. George’s evaluation revealed that she required maximum assistance for the completion of her therapy goals.

Ms. George appreciated the care she received from her nurses, Jessa and Jacinta, and built a close relationship with them during her stay. Ms. George also enjoyed being visited by Omar Garcia from the recreation department and worked together with him on artwork during his visits.

Two weeks into her stay, Ms. George had made significant strides in her rehabilitation journey. Ms. George was able to complete her bed mobility tasks with minimal assistance, was performing ADL tasks such as dressing with moderate assistance, and could walk 25 feet with a rolling walker with moderate assistance.

Ms. George always had a pleasant demeanor and greeted everyone with a smile. Ms. George enjoyed the luxury amenities at Sans Souci like their signature Snack Cart Program provided by the community Concierge and daily newspaper delivery by the recreation team.

Ms. George continued to make significant strides throughout her stay. She was upgraded from utilizing 3 liters of portable oxygen to 2 liters of oxygen by a respiratory therapist, Terrence Seymour, one month into her stay. Ms. George could now ambulate 90 feet with a rolling walker with minimal assistance, ascend and descend 4 steps on the community stairs with moderate assistance, and could perform ADL’s and transfers with minimal assistance.

The week prior to discharge, Ms. George was able to perform bed mobility tasks and functional transfers independently. Ms. George was able to ambulate 350 feet, ascend and descend 15 steps, and perform ADL’s independently. At discharge, Ms. George shared “thank you to all the staff that assisted in caring for me. I came to Sans Souci on my back and now I am leaving walking again. I appreciate all that everyone at Sans Souci has done for me and for helping me get better!”

Details of Experience:
Mr. Reyes arrived at Sans Souci Rehabilitation and Nursing Center on August 12, 2019, from St. Joseph’s Medical Center. Prior to arriving at Sans Souci, Mr. Reyes complained of muscle weakness and difficulty walking. Mr. Reyes underwent a total knee replacement to correct his knee joint. Upon completion of his hospital stay, Mr. Reyes was referred to Sans Souci for physical rehabilitation in hopes to begin walking again.

Upon admission, Mr. Reyes was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Mr. Reyes to our community and completed evaluations to custom tailor Mr. Reyes’s stay.

Mr. Reyes was evaluated by members of the rehabilitation team, Dexter Galette, certified occupational therapy assistant, and Samuel Ignacio, doctor of physical therapy, who performed initial assessments. Mr. Reyes’s initial rehabilitation goals were to be able to safely perform transfers independently, ambulate 250 feet with a rolling walker and improved range of motion, independently perform ADL’s, and ascend and descend 15 steps on stairs.

Mr. Reyes’s evaluation revealed that he was able to ambulate 25 feet using a rolling walker with supervision and required minimal assistance with functional transfers. Mr. Reyes was unable to ascend or descend stairs and required minimal assistance in completing ADL tasks.

During his stay, Mr. Reyes became close friends with his roommate who was also fluent in Spanish. Mr. Reyes enjoyed attending recreation activities, such as bingo, BBQs, and musician visits led by Recreation Director, Leduc Le and Recreation Therapist, Omar Garcia.

A week and a half into his stay, Mr. Reyes met his occupational therapy goals. He was now able to complete activity of daily living (ADL) tasks independently and could perform functional transfers with modified independence. Mr. Reyes made significant progress with his physical therapy goals as well! He was now able to ambulate 225 feet utilizing a rolling walker with supervision, and ascended and descended four steps on the community stairs.

Mr. Reyes shared with the community’s concierge that he was a veteran of the U.S. Army. Mr. Reyes enjoyed attending the monthly Veteran’s Coffee Social with other residents as well as staff veterans at Sans Souci led by our concierge, Aryeh Schachter.

Three weeks into his stay, the facility began planning Mr. Reyes’s discharge home. Mr. Reyes exceeded his ambulation goal and was now able to ambulate unlimited distances with a rolling walker independently. Mr. Reyes met his stair goal and was able to ascend and descend 15 steps on the community stairs with handrails.

Upon the time of discharge, Mr. Reyes shared “My stay at San Souci was a great experience. I went in with pain and came out pain-free. The staff took great care of me. The rehab team was great! I would recommend this place to anyone!”

Details of Experience:
Ms. Christine Kalsky arrived at Sans Souci Rehabilitation and Nursing Center on June 25, 2019, from St. Joseph’s Medical Center. Prior to arriving at Sans Souci, Christine developed pneumonia, sustained a pulmonary embolism, all with a prior diagnosis of COPD. Upon completion of her hospital stay, Christine was referred to Sans Souci to begin respiratory therapy, as well as physical and occupational therapy.

Upon admission, Christine was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Christine to our community and conducted evaluations to custom tailor Christine’s stay. Christine required 3 liters of oxygen and utilized a wheelchair at the time of admission.

Christine was evaluated by members of the rehabilitation team who performed an initial evaluation on Christine. Christine’s rehabilitation goals were to be able to safely perform transfers independently, ambulate 275 feet with a rolling walker, perform ADL’s independently, and ascend 15 steps on stairs.

During her stay, Christine was an active participant in the daily recreation activities. She made many friends among the staff as well as residents. Christine enjoyed attending recreation activities such as horse racing, birthday parties, bingo, BBQs, and musician visits led by Recreation Director, Leduc Le, and Recreation Therapist, Omar Garcia. Christine also enjoyed the luxury concierge amenities here at Sans Souci, such as our signature Snack Cart program for residents. Christine would alert the concierge, Aryeh Schachter, who would accommodate all of her expressed wishes whenever she had a special request.

Christine also began working with our Respiratory Therapist, Terrance Seymour for pulmonary rehabilitation education. Christine was given education on how to manage her diagnosis and given exercises to increase endurance and strength with limited use of oxygen. With exercise, Christine was soon able to only necessitate 2 liters of oxygen.

Three weeks into her stay, Christine was upgraded to utilizing only 1 liter of oxygen. Christine worked with Dexter Galette from the OT team and Sam Opeyemi from the PT to meet her OT and PT goals. By mid-July, Christine was able to ambulate 150 feet with minimal assistance using a rolling walker, ascend 8 steps on the stairs, and required only minimal assistance for her ADL’s as well as transfers.

Christine’s progress with decreasing portable oxygen intake continued to improve. Terrance had Christine utilizing only room air during the week and as needed portable oxygen. One week later, Christine was taken off portable oxygen altogether and was able to thrive in the community.

By the end of July, Christine continued to make progress with her rehab goals. With the team’s support, Christine was now able to walk 200 feet with a rolling walker and ascend 14 steps on the stairs.

At her discharge at the beginning of August, Christine had exceeded her PT goals for ambulation and was able to walk a distance of 300 feet independently using a rolling walker and ascend more than 15 steps on the community stairs utilizing handrails. Christine also met her OT goals and was independently performing transfers and ADL’s.

Christine shared with the team as she was leaving the community, “Sans Souci gave me purpose and meaning to my life. If it wasn’t for them, I wouldn’t be so motivated to keep pushing on. They have activities daily, which helped my mental status. Every day here was great. Thank you for all the love and support and for making me feel like family. If you need rehabilitation and nursing care, this is the place to go!”

Details of Experience:
Mr. Collazo arrived at Sans Souci Rehabilitation and Nursing Center on April 4, 2019, from Montefiore Medical Center. Prior to coming to Sans Souci, Mr. Collazo had a high white blood cell count stemming from a wound to his lumbar back. Upon completion of his hospital stay, Mr. Collazo was referred to Sans Souci to begin receiving wound care and physical rehabilitation.

Upon admission, Mr. Collazo was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Mr. Collazo to our facility and conducted evaluations to custom tailor Mr. Collazo’s stay. Mr. Collazo required a wound vac to heal the skin wound he sustained prior to admission. At the initial evaluation by the wound nurse, Mr. Collazo’s wound was 13 cm.

Mr. Collazo began working with our Physical Therapist, Sam Beloni, and Occupational Therapist, Dexter Gallate, to begin working on bed mobilities, transfers, gait training, and performance of ADL’s. Mr. Collazo was bed-bound at the time of admission. Upon evaluation, Mr. Collazo was totally dependent on transfers, bed mobilities, and ADL’s.

At the beginning of June, Mr. Collazo’s wound had almost entirely healed. The wound was 1.5 cm and he no longer required a wound vac for healing.

By the middle of June, Mr. Collazo was able to perform bed mobilities and transfers with supervision. Mr. Collazo was able to ambulate with a wheelchair. At the end of June 2019, Mr. Collazo was able to perform ADL’s with minimal assistance.

During his stay, Mr. Callazo was an active participant in the daily recreation activities. He especially enjoyed attending recreation activities with Recreation Director, Leduc Le, and Recreation Therapist, Omar Garcia. Mr. Collazo is a very social individual and an avid sports fan, he enjoyed speaking with our Director of Concierge Services, Aryeh Schachter, and Maintenance Technician, William Dameus, about the latest news in sports.

Mr. Collazo enjoys the outdoors and prior to his admission worked in landscaping. Mr. Collazo enjoyed going out to the community patio and observing the trees, plants, and flowers on the grounds.

At the beginning of July 2019, Mr. Collazo was able to walk using Parallel Bars for 20 feet with moderate assistance. Within two weeks of using the Parallels Bars, Mr. Collazo began training with a rolling walker using assistance from two staff.

Upon discharge, Mr. Collazo had made significant progress in his physical rehabilitation. Mr. Collazo’s strength, endurance, and mobility had drastically since his arrival at Sans Souci. Mr. Collazo will be continuing his healing journey at an outpatient physical rehabilitation facility.

Ms. Kennedy arrived at Sans Souci Rehabilitation and Nursing Center on January 18, 2019 post-discharge from St. John’s Riverside Hospital. Prior to coming to Sans Souci, the patient experienced episodes of shortness of breath, due to her multiple diagnoses of essential hypertension, COPD, and asthma. Upon completion of her hospital stay, Ms. Kennedy was referred to Sans Souci to receive respiratory therapy.

Upon admission, Ms. Kennedy was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Ms. Kennedy to our facility and conducted evaluations to custom tailor the patient’s stay. Ms. Kennedy required 3 liters of oxygen at the time of admission.

Ms. Kennedy began working with our Respiratory Therapist, Terrance Seymour, for pulmonary rehabilitation education. Upon evaluation, Ms. Kennedy was able to walk a distance of 150 feet without an assistive device, with two rest periods.

Throughout her stay, Ms. Kennedy was determined to limit the use of portable oxygen. She was motivated to do her exercises and continuously progressed. By mid-March, was utilizing only 2 liters of portable oxygen. She was able to walk 600 feet with a small two minute rest periods every 200 feet.

During her stay, this patient was an active participant in daily recreation activities. She especially enjoyed attending horse racing, birthday parties, bingo, and musician visits with Recreation Director, Leduc Le, and Recreation Therapist, Omar Garcia. Ms. Kennedy also enjoyed the luxury amenities at Sans Souci, such as; our signature Snack Cart program for residents.

Towards the end of April, Ms. Kennedy’s endurance and strength had improved significantly. She was now utilizing only 1 liter of oxygen and was able to ambulate 800 feet with limited rest periods. Upon discharge, Ms. Kennedy was able to ambulate independently without the assistance of portable oxygen. Ms. Kennedy shared that she enjoyed her time at Sans Souci and that she is thankful for the nursing and rehab staff motivating her to reach her therapy goals.

Mr. James Adams arrived at Sans Souci Rehabilitation and Nursing Center on April 19, 2019, from an inpatient hospitalization at New York-Presbyterian Hudson Valley Hospital. Mr. Adams was admitted to Sans Souci with a primary diagnosis of cellulitis with a wound on the right residual surgical stump from his right lower extremity below-knee amputation.

Mr. Adams was greeted by multiple professionals of the Sans Souci interdisciplinary team who had anticipated his arrival. Within the first 2 hours, he was greeted by clinicians, therapists, physicians, administrative staff, and many other valued members of the Sans Souci team to welcome him and answer any questions that he may have had.

Mr. Adams was evaluated by the Sans Souci clinical team to assess his wound and begin treatment. Upon initial evaluation, Mr. Adams’s wound was 2 inches in length, 1.5 inches in width, and 1.5 inches in depth. Mr. Adams was administered silver alginate for a period of three weeks to begin healing the wound.

During his stay, Mr. Adams was an active participant in the Sans Souci recreation department’s daily activities and enjoyed the luxury amenities at Sans Souci, such as their signature Snack Cart Program. Mr. Adams forged a close relationship with Leduc Le, Director of Recreation at Sans Souci, Omar Garcia, Recreation Therapist, and Aryeh Schachter Director of Concierge Services. Mr. Adams became friends with many residents and staff members throughout his stay.

One month into his stay, Mr. Adams’s wound had decreased in size to a 1.2-inch width and .5 inch depth.

Having completed a prior stay at a sister community in the network, Mr. Adams was familiar with the signature CareRite Centers Comprehensive Amputee Rehabilitation Program led by Todd Schaffhauser and Dennis Oehler. Mr. Adams utilized the support program for motivation, support, and inspiration for his bilateral below-the-knee amputation.

Mary Conte, RN Unit Manager, worked with Mr. Adams to educate him regarding his wound care. Mr. Adams was trained on how to self treat the wound for his upcoming discharge home. Upon discharge, Mr. Adams’s wound had shrunk to a size of .5 inches in length and a .2 inch depth. Prior to leaving Sans Souci, Mr. Adams shared that “Sans Souci Rehab is a great place to receive nursing care. Throughout my stay, the staff at Sans Souci have been very helpful and taken great care of me. I wish I could stay here, but I’ll come back and visit soon!”

Mr. Jackson arrived at Sans Souci Rehabilitation and Nursing Center on March 4th, 2019 from an inpatient hospitalization at Saint John’s Hospital. Mr. Jackson was presented with a diagnosis of spinal fusion, muscle weakness, and difficulty walking. When he first arrived, he needed the use of a wheelchair for ambulation.

Mr. Jackson was greeted by multiple professionals of the Sans Souci interdisciplinary team who had anticipated his arrival. Within the first 24 hours, he was greeted by clinicians, therapists, physicians, administrative staff, and many other valued members of the Sans Souci team to welcome him and answer any questions that he may have had.

Mr. Jackson began working with his physical therapist, Sam, and occupational therapist, Diane to improve ambulation and completion of daily activities. Mr. Jackson began his rehab requiring total dependence for bed mobilities and ambulation, necessitating him to use a wheelchair to ambulate.

During his stay, Mr. Jackson could always be seen with a positive attitude and conversing with staff. Mr. Jackson is an avid sports fan who reads, watches and listens to all sporting programs. Mr. Jackson would regularly discuss current sporting news at length with Sans Souci’s Director of Concierge Services, Aryeh. Ms. Jackson also enjoyed the luxury amenities at Sans Souci such as their signature Hot Towel and Snack Cart program for residents.

By the beginning of April, Mr. Jackson was able to complete bed mobility tasks with minimal assistance, ambulate 175 feet supervised using a rolling walker, and was able to ascend 12 steps on the stairs. Mr. Jackson also progressed and was able to complete functional activities such as dressing himself.

Upon discharge, Mr. Jackson was able to complete all bed mobility tasks independently, ambulate 300 feet using a rolling walker, and ascend 15 steps on the stairs. Before leaving Sans Souci, Mr. Munoz stated: “I have respect for all the staff at Sans Souci and the work they do.”

Mr. Munoz was greeted by multiple professionals of the Sans Souci interdisciplinary team who had anticipated his arrival. Within the first 24 hours, he was greeted by clinicians, therapists, physicians, administrative staff, and many other valued members of the Sans Souci team to welcome him and answer any questions that he may have had.

Mr. Munoz began working with his physical therapist, TK, and occupational therapist, Dexter, to improve ambulation and completion of daily activities. Mr. Munoz began his rehab requiring max assistance for bed mobilities and total dependence with ambulation requiring him to use a wheelchair in order to ambulate.

During his stay, Mr. Munoz was an active participant in our veterans programming. Mr. Munoz attended the community monthly Veteran’s Coffee Social led by Sans Souci’s Director of Concierge Services, Aryeh. Mr. Munoz shared stories of his time working as a firefighter for the U.S. Air Force with our other Veterans in attendance.

By the end of February, Mr. Munoz was able to ambulate 200 feet supervised using a rolling walker and he was able to ascend 4 steps on the stairs. Mr. Munoz also progressed and was able to complete functional activities such as dressing himself with minimum assistance!

Mr. Munoz especially enjoyed attending the special recreation events led by our recreation therapists, Omar and Topeka. Mr. Munoz was a regular participant in events such as painting, pizza making, bingo, and the St. Patrick’s Day celebration. Ms. Munoz also enjoyed the luxury amenities at Sans Souci such as their signature hot towel and snack cart program for residents.

Upon discharge, Mr. Munoz was able to ambulate 250 feet using a rolling walker and ascend 6 steps on the stairs. Mr. Munoz was safely able to perform bed mobilities and daily activities such as grooming and dressing. Prior to leaving Sans Souci, Mr. Munoz stated “I had a great stay at Sans Souci!

Ms. Rice arrived at Sans Souci Rehabilitation and Nursing Center on January 5, 2019, from St. Joseph’s Hospital. Prior to coming to Sans Souci, Ms. Rice sustained a fracture to her right hip while ambulating in the community. Upon completion of her hospital stay, Ms. Rice was referred to Sans Souci to begin rehabilitation for her hip.

Upon admission, Ms. Rice was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Ms. Rice to our community and conducted evaluations to custom tailor Ms. Rice’s stay. Ms. Rice required total assistance with ambulation and many of her daily activities. Ms. Rice also required the use of a wheelchair.

During her stay, Ms. Rice was an active participant in the daily recreation activities. She especially enjoyed attending the special recreation events including painting, birthday celebrations, and musician visits. Ms. Rice also enjoyed the luxury amenities at Sans Souci such as their signature hot towel and
snack cart program for healing patients.

Ms. Rice began working with one of our occupational therapists, Shani and physical therapist, T.K. to improve her balance, mobility, and overall strength and endurance in order to be able to complete routine tasks such as ambulation and self-care. Ms. Rice was unable to walk or use grab bars at the time of her initial evaluation.

By the fifth week of her stay, Ms. Rice was able to use grab bars independently. By the 7th week of her stay, Ms. Rice was able to navigate 250 feet with a rolling walker and ascend and descend 15 stairs with minimal assistance!

Upon discharge, Ms. Rice was able to navigate 250 feet with a rolling walker as well as ascend and descend 15 stairs utilizing handrails independently. Ms. Rice shared during her stay, “the staff at Sans Souci have been amazing, and the facility is a great place!”

How did this patient hear about Sans Souci? Previous admission

Ms. Daysi Carranza arrived at Sans Souci Rehabilitation and Nursing Center from St. John’s Riverside Hospital on December 31, 2018. Ms. Carranza recently underwent a total hip replacement and had a wonderful therapy experience during a previous admission to Sans Souci over a year ago. Due to her right hip flaring up resulting in a hospitalization, she decided to return to Sans Souci to receive short term rehabilitation.

Upon admission, Ms. Carranza was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Ms. Carranza back and conducted evaluations to custom tailor Ms. Carranza’s stay and ensure she would receive incredible levels of care, comfort, and customer service she experienced during her previous admission. Ms. Carranza necessitated the use of a wheelchair for ambulation and was able to complete most daily activities with minimal to moderate assistance.

During her stay Ms. Carranza was an active participant at the daily recreation activities. She especially enjoyed attending the special recreation events such as musician visits and the monthly resident birthday celebration. Ms. Carranza developed a special relationship with our recreation therapist, Omar.

Ms. Carranza began working with our occupational therapist, Shaniqua and physical therapist, Carrie to improve her balance, mobility, and overall strength in order to be able to complete routine tasks such as ambulation and self care. Within a couple weeks of rehab at Sans Souci, Ms. Carranza demonstrated significant improvement in strength and endurance, and required a rolling walker for ambulation.

At discharge, Ms. Carranza was able to navigate stairs, ascending 30 steps utilizing handrails. Ms. Carranza was able to ambulate unassisted an unlimited distance using a rolling walker. Ms. Carranza stated upon discharge, “I am very happy with all the staff at Sans Souci. The therapy was excellent. They are very attentive to my needs. I truly enjoyed my stay.”
