Details of Experience:
Ms. Dorothy was admitted to beautiful Sans Souci on May 4, 2021, with a diagnosis of increasing swelling of the lower extremities, which led to Ms. Dorothy’s difficulty with normal mobility and function. Right from the start, Ms. Dorothy was committed, excited, and eager to start physical therapy…right away. The following morning, Ms. Dorothy was visited by our amazing Rehab Team, who evaluated Ms. Dorothy and together planned long-term goals for the patient to achieve.  With no time to waste the following morning, May 5, Ms. Dorothy started on her beautiful rehabilitation journey.  

Upon admission, the patient was unable to perform basic things like brushing her teeth; she was unable to ambulate on more than 10 feet and with maximum assistance.  Ms. Dorothy could not stand and balance on her own for a while and perform bed mobility, meaning sitting up or getting in or out of bed by herself safely due to weakness and unsteadiness. 

Ms. Dorothy’s goals were to complete, by herself, hygiene and grooming tasks while standing in front of the mirror, with modified independence for safety awareness, and to be able to safely perform toileting tasks using a raised toilet seat commode for better assistance. Ms. Dorothy would be able to perform safely and efficiently dressing with the use of a dressing stick and a sock aid stick and perform functional transfers with zero assistance, and safely ambulate on level surface 125 feet using her roller walker.

As the days passed, Ms. Dorothy showed great strength and eagerness. She progressed and achieved the goals that were set from day one.  The patient’s determination, along with the help of our phenomenal Rehabilitation team, achieved her goals, even before their set dates.

With all the significant progress Ms. Dorothy exhibited, the Rehab Team, together with our devoted Social Workers, determined that it was time to plan for Ms. Dorothy’s discharge date. Together they reevaluated Ms. Dorothy and decided on May 21, 2021 (just 17 days from Ms. Dorothy’s initial admission), the patient would be discharged back home. Ms. Dorothy was very excited to finally see her dog again, which she had missed so much. 

Upon discharge, Ms. Dorothy was able to complete hygiene and grooming tasks while standing in front of the mirror with modified independence for safety awareness, perform toileting tasks using a raised toilet seat commode for better assistance, safely and efficiently dress with the use of a dressing stick and a sock aid stick.  Ms. Dorothy performed functional transfers with zero assistance and safely ambulated on level surface 150 feet using her roller walker (25 additional feet from the initial goal!).

As our team at Sans Souci always does… post-discharge, we followed up with Ms. Dorothy and were pleased to hear that she was doing great and is enjoying her time home with her beautiful dog!

Details of Experience:
On March 19, 2021, Ms. Eileen Ochart was admitted to beautiful Sans Souci with a diagnosis of spontaneous hemothorax, which is the extraction of fluid in the chest section; as well as fractured ribs on Ms. Ochart’s left side after experiencing a bronchitis episode, which is basically persistent coughing, wheezing and shortness of breath.

The Sans Souci team welcomed Ms. Ochart and made sure she had a smooth admission process. Ms. Ochart was visited by our wonderful Rehab Team, including our physical therapist and occupational therapist. At that time, they assessed Ms. Ochart and created a plan with long-term goals.

Upon Admission, Ms. Ochart was weak and was continuously coughing and wheezing. She was unable to climb stairs and was hardly able to sit or stand by herself, and only able to walk 100 feet, at best. The patient’s goals were to successfully and independently be able to perform bed mobility tasks, to complete sit to stand transfers including all variety of moving in and out of chair settings, to ambulate 200 feet without using any additional help or assistance, and to ascend and descend stairs with assistance of railing.  Ms. Ochart was determined and self-driven; she was excited to begin immediately. Taking her lead and wasting no time, the Rehab Team began therapy the very next day.

Unfortunately, week two arrived, and Ms. Ochart was tested positive for Covid 19. The news was a setback for Ms. Ochart and the staff at the same time. Being positive meant the Rehab Team only had so much they were able to do together with the patient. It was extremely difficult. To make matters worse, since rehab was somewhat put to a hold, Ms. Ochart’s mobility began to decline from walking 100 feet to only being able to walk 60 feet, much the same with her bed mobility.

Those 14 isolation days were tough but eventually came to an end. With no time to waste, Ms. Ochart, just like when she was first admitted, was determined and driven to continue, and so the Rehab Team and Ms. Ochart did. Ms. Ochart kept progressing, from 50 feet to 75 feet, and she kept climbing and climbing. The Rehab team was very happy with Ms. Ochart’s progress and began talking and planning with Ms. Ochart for her discharge date. Together, they decided on April 19, 2021. In the days leading up to discharge, Ms. Ochart was doing phenomenally. She was able to sit and stand up by herself, walk and even climb stairs by herself. April 19 arrived, and Ms. Ochart was discharged home. 

Upon Discharge, the patient was successfully and independently able to perform bed mobility tasks, complete sit-to-stand transfers, which include getting in and out of chair settings, and independently ambulate unlimited distance without using any additional help or assistance.  Ms. Ochart was successfully and safely able to ascend and descend stairs with the aid of railing up to 15 steps.

As our team at Sans Souci always does post-discharge, we followed up with Ms. Ochart and were pleased to hear that she is doing great and was thankful for her incredible experience with us at Sans Souci!

Details of Experience:
Mrs. Josefina G. arrived at Sans Souci Rehabilitation and Nursing Center on May 28, 2020 from St. John’s Riverside Hospital. Prior to coming to Sans Souci, Mrs. G. had been diagnosed with COVID-19, UTI, polyneuropathy, respiratory failure, experienced muscle weakness, and difficulty walking. Also, prior to her hospital stay, Mrs. G. was living independently with her family and required no assistive devices. Mrs. G. was referred to Sans Souci to begin receiving PT, OT, and nursing care.

Upon admission, Mrs. G. was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Mrs. G. to our community and conducted evaluations to custom tailor Mrs. G.’s stay. Mrs. G. was non-ambulatory at the time of admission and required a wheelchair.
Mrs. G. began working with our Physical Therapist, Cynthia Antoine, DPT, and Occupational Therapist, Cheryl Matthews, OTR/L, to regain functional independence.

Josefina was nervous at the beginning of her stay that she would never fully recover and be able to walk and stand again. The Sans Souci interdisciplinary team worked to motivate Josefina that she had the capability to return to her prior functioning level. The staff moved Josefina to a room with a larger window, where her family was able to visit regularly and provide words of motivation and inspiration in chalk on the concrete path her room overlooked.

Upon evaluation, Mrs. G. required total dependence with standing, ambulation, transfers, and activities of daily living. Mrs. G.’s initial physical therapy short term goals were to complete bed mobilities with moderate assistance, functional transfers with total dependence, and ambulate 10 feet on level surfaces using a rolling walker. Her long-term goals were to complete bed mobilities supervised, perform functional transfers with contact guarding assistance, and ambulate 100 feet using a rolling walker with minimal assistance. Mrs. G.’s occupational therapy goals were to perform eating tasks, dressing, grooming, and ADL’s with maximum assistance. Josefina received wound care to treat the ischemia to her fingers. The nursing care provided helped heal Mrs. G.’s wounds and allowed her full use of her hands again.

One month into her stay, Josefina was upgraded to requiring moderate assistance for bed mobilities. She could ambulate 10 feet with a rolling walker and moderate assistance and perform functional transfers using contact guarding assistance. Josefina also was upgraded to moderate assistance for most ADL’s. Mrs. G. was motivated by the staff and her family to persevere. She developed a very close relationship with our recreation therapist, Omar Garcia, who would visit her daily to provide entertainment, a listening ear, and reassurance that she would be able to make a full recovery.

Two months into her stay, Mrs. G. had made significant strides in her therapy. Josefina was now able to perform bed motilities with moderate independence. She could perform functional transfers with supervision and could ambulate 50 feet on level surfaces using a rolling walker. Mrs. G. was also upgraded to minimal assistance for most ADL’s. By the beginning of August, Mrs. G. had progressed so much that she could now ambulate distances of 150 feet with supervision. The rehab team began stair training with Josefina. She safely navigated 12 steps on the community stairs with stand by assistance. Two weeks prior to discharge Josefina was ambulating distances of 300 feet with a rolling walker and began training with a cane.

The week prior to discharge Josefina was ambulating unlimited distances with a rolling walker and could use a cane for ambulating shorter distances. She could perform all bed mobilities, functional transfers, and ADL’s independently. Upon discharge Josefina shared, “every staff member at Sans Souci motivated me to keep pushing forward. I came in unable to move and mostly bed bound and am leaving here walking! The happiest moment was walking again for the first time as it was then that I knew I could accomplish anything and would return to my old self. Thank you to everyone that works at Sans Souci from housekeeping, dietary, nursing, recreation, and rehab everyone was truly amazing and motivated me to keep going and moving forward!”

Details of Experience:
Mr. Tiburcio arrived at Sans Souci Rehabilitation and Nursing Center on May 26, 2020, from St. Joseph’s Medical Center. Prior to coming to Sans Souci, Mr. Tiburcio experienced difficulty breathing and shortness of breath. Mr. Tiburcio was admitted to status post-COVID-19 from the hospital and was oxygen dependent, as a result. Mr. Tiburcio was referred to as Sans Souci to begin receiving PT, OT, respiratory therapy, and nursing care.

Upon admission, Mr. Tiburcio was greeted by members of the Sans Souci interdisciplinary team. The team welcomed Mr. Tiburcio to our community and conducted evaluations to custom tailor the patient’s stay. Mr. Tiburcio required a wheelchair for ambulation at the time of admission.

Prior to his hospital stay, he was living independently and required no assistive devices. Mr. Tibucio began working with our Physical Therapist, Danilo Regala, DPT, and Occupational Therapist, Cheryl Matthews, OTR/L, to regain functional independence. He also began working with our respiratory therapist, Terrence Seymour, to begin the process of weaning off oxygen and utilizing room air for breathing. The patient required 15 liters of oxygen at the time of admission.

Upon evaluation, Mr. Tiburcio required supervision with a 40-foot distance in ambulation with a rolling walker, total dependence navigating stairs, moderate assistance with bed mobilities, and ADL’s (activities of daily living). Mr. Tiburcio’s therapy goals were to be able to perform bed motilities, ADL’s, functional transfers, navigate four stair steps on the community stairs, and walk a distance of 300 ft. without requiring assistance.

After two weeks of working with respiratory therapy, Mr. Tiburcio only required 10 liters of oxygen. Mr. Tiburcio reacted well to the nursing care received and began to feel better. He could perform wheelchair propulsion of 300 feet without exertion for 5 minutes. He was able to use a rolling walker to ambulate distances of 150 feet, navigate 3 steps on the stairs, and was upgraded to minimal assistance with many ADL’s.

During his stay, Mr. Tiburcio was an active participant at our daily recreation programming with Recreation Therapist, Omar Garcia. One month into his stay, Mr. Tiburcio was upgraded to requiring only four liters of portable oxygen. Mr. Tiburcio continued to progress with his ADL’s and was now able to ambulate a distance of 300 feet with a rolling walker with modified independence. The patient could now ascend and descend 15 steps on the community stairs without experiencing shortness of breath, using modified independence. One week prior to discharge, Mr. Tiburcio had made significant strides in his respiratory therapy sessions. He now breathed utilizing only room air. Mr. Tiburcio continued to make progress with his OT and PT sessions and was able to safely ambulate unlimited distances, navigate stairs, and perform ADL’s independently.

Upon discharge, Mr. Tiburcio shared, “I was sent to recover at Sans Souci Rehabilitation and Nursing Center, after beating the Coronavirus. It left my lungs severely inflamed and gave me two blood clots. The medical staff, nursing staff, PT, OT, respiratory staff, and housekeeping department all made sure that my stay and recovery was as fast and pleasant as possible. Sans Souci provided me specialized and individualized treatment and care. This center is like the Taj Mahal of all rehab centers and I would highly recommend it to others.”

Details of Experience:
Mr. Anthony B. arrived at Sans Souci Rehabilitation and Nursing Center on May 29, 2020, from an inpatient hospital, stay at Burke Rehabilitation Center. Prior to his arrival at Sans Souci, Mr. B. experienced a stroke with right side chronic middle cerebral artery (MCA) occlusion w/ left side hemiparesis. As a result, Mr. B. has decreased coordination, decreased strength, and decreased activity tolerance inhibiting ADL performance. Mr. B. was referred to Sans Souci to begin receiving PT, OT, and nursing care to begin his healing journey to return to the community.

Upon admission, Mr. B. was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Mr. B. to our facility and conducted evaluations to custom tailor Mr. B.’s stay. He also required a rolling walker for ambulation upon admission.

Prior to his hospital stay, Mr. B. resided at his own residence and ambulated unlimited distances, independently, without the use of assisted devices. He began working with our Physical Therapist, Carre Peters-Smith, DPT, and Occupational Therapist, Kecia DePass, COTA, to return to independence with ADL’s and ambulation.

Upon evaluation, he also required minimal assistance with ADL’s, such as bed mobilities, functional transfers, grooming, and dressing. Mr. B. was able to ambulate with a rolling walker a distance of 40 feet, using minimal assistance.

His therapy goals were to be able to perform bed mobilities and functional transfers with modified independence, independently perform ADLs, such as grooming and dressing, and ambulate distances 350 feet using a quad cane.

After two weeks of working with the therapy department, Mr. B. was able to ambulate 150 feet using a quad cane with set-up assistance and safely performed grooming and dressing with contact guard assistance. His long term therapy goal was upgraded to ambulating unlimited distances with a quad cane.

Mr. B. was a very outgoing and friendly resident during his stay. He forged many relationships with staff and residents, alike. He enjoyed the luxury Sans Souci amenities, such as the complimentary Starbucks coffee offerings from the community Starbucks Serenade Brewer and various weekly luxury refreshments from our signature snack cart program.

By the third week of his stay, Mr. B. could ambulate 250 feet utilizing a quad cane with modified independence. He was upgraded to set-up level assist with functional transfers and bed mobilities. Mr. B. was also upgraded to contact guard assistance with most ADL’s.

The week of his discharge, Mr. B. was upgraded to independently performing bed mobilities and functional transfers. He could ambulate unlimited distances with a quad cane independently and was upgraded to stand by assistance for grooming tasks and dressing ADL’s!

On June 25th, Mr. B. discharged from Sans Souci. Upon discharge, he shared, “The rehab department at Sans Souci is amazing. Kecia and Carrie were fantastic to work with and helped me regain my strength. The entire nursing department took great care of me. Thank you to my nurses Emily and Jessa and CNA’s Ruth, Pauline, and Albania who helped me recover. I really enjoyed my stay at Sans Souci.”

Details of Experience:
Mr. Justo Santiago arrived at Sans Souci Rehabilitation and Nursing Center on January 17, 2020 from an inpatient hospital stay at St. John’s Riverside Hospital. Prior to coming to Sans Souci, Justo experienced hypertension, an acute gout flare up, generalized weakness, and impairments in his muscle strength. Mr. Santiago was referred to Sans Souci to begin receiving PT, OT, and skilled nursing care to begin his healing journey, with the goal of returning to the community.

Upon admission, Mr. Santiago was welcomed by multiple members of the Sans Souci interdisciplinary team. The team conducted evaluations to ensure Mr. Santiago would have a very productive stay. During those evaluations, the team learned Mr. Santiago is a retired NYC policeman.

Prior to his hospital stay, Justo was living independently and required a rolling walker for ambulation and a cane at home. Mr. Santiago’s home required two steps to enter. Upon admission, he required a wheelchair for ambulation. Mr. Santiago began working with our Physical Therapist, Danilo Regala, DPT and Occupational Therapist, Lauren Wendeboom, OTR/L. Upon evaluation, he required minimal assistance with bed mobility and functional transfers. Justo was fully dependent on assistance for stair navigation, moderate assistance with ambulation, and maximum assistance with Activities of Daily Living (ADLs).

Mr. Santiago’s Physical Therapy goals were to be able to perform functional transfers and bed mobility independently, safely ascend/descend 12 stairs utilizing handrails, and ambulate distances of 100 feet utilizing a rolling walker. Mr. Santiago’s Occupational Therapy goals included regaining full independence with ADL’s. After two weeks working with our rehabilitation team, Mr. Santiago was now able to perform bed mobility with contact guarding assistance, ambulated 25 feet using a rolling walker with minimal assistance and ambulated 4 steps with maximum assistance. Mr. Santiago was now able to complete his ADLs independently.

During his stay, Mr. Santiago was an active participant at the daily recreation activities. Justo was very social and became friends with many staff members and residents. He could always be found with a smile on his face throughout his stay. He especially enjoyed Nerf gun target practice where he was able to practice his skills from his days on the police force. He also participated in the community’s art sessions and weekly coffee social with our Recreation Therapist Omar Garcia. Mr. Santiago also enjoyed the amenities at Sans Souci such as the Snack Cart and the Starbucks Serenade Brewer.

After four weeks, Mr. Santiago could perform bed mobility and functional transfers with supervision. Mr. Santiago could safely navigate 6 steps on the community stairs with minimal assistance, ambulate 50 ft with a rolling walker, and complete ADLs independently.

At the end of his stay, Mr. Santiago was able to ambulate 150 ft with a rolling walker, navigate 12 steps on the community stairs and perform all transfers and bed mobility independently. Mr. Santiago shared, “Sans Souci has been a positive experience for me. The staff has been very kind and attentive to my needs throughout my stay. I thank everyone who took care of me during my stay.”

Details of Experience:
Mr. William Schillinger arrived at Sans Souci Rehabilitation and Nursing Center on July 8, 2019, from New York-Presbyterian Hospital. Prior to coming to Sans Souci, he experienced episodes of shortness of breath due to his prior deep vein thrombosis (DVT), chronic hypertension, and asthma. Upon completion of his hospital stay, Mr. Schillinger was referred to Sans Souci to begin receiving PT, OT, and nursing care.

Upon admission, Mr. Schillinger was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Mr. Schillinger to our facility and conducted evaluations to custom tailor Mr. Schillinger’s stay. Mr. Schillinger required a wheelchair for ambulation at the time of admission.

Prior to his hospital stay, Mr. Schillinger was living independently and required no assistive devices. Mr. Schillinger’s home required 3 steps to enter. Mr. Schillinger began working with our Physical Therapist, Sam Ignacio and Occupational Therapy with our Assistant Director of Rehabilitation, Vinny Carvhalo COTA. Upon evaluation, Mr. Schillinger required total dependence with ambulation, bed mobilities, and ADL’s. Mr. Schillinger’s therapy goals were to be able to perform bed motilities, ADL’s, functional transfers, navigate 3 steps on the community stairs, and walk a distance of 250 ft without requiring assistance.

After two weeks working with our therapy department, Mr. Schillinger was upgraded to requiring moderate assistance with bed mobilities, max assistance with ADL’s and functional transfers. Mr. Schillinger was still unable to ambulate or navigate stairs due to pain.

During his stay, Mr. Schillinger was an active participant in the daily recreation activities. He especially enjoyed attending recreation activities such as horse racing, musician visits, birthday parties, bingo, art sessions, and coffee social with our Recreation Therapist Omar Garcia. Mr. Schillinger also enjoyed the luxury amenities at Sans Souci such as their signature Snack Cart and Hot Towel programs for residents.

By the end of August, Mr. Schillinger continued to make significant strides in his road to recovery. Mr. Schillinger was now able to safely perform two- hours of activity without experiencing pain or distress, functional transfers, and most ADLs with moderate assistance. At the beginning of September, Mr. Schillinger was able to navigate the community parallel bars with minimal assistance.

Mr. Schillinger was very social and became friends with many residents in his unit. Mr. Schillinger always wanted to help others and participated in assembling care packages for a local homeless shelter during our annual Day of Service for the local Yonkers community.

At the beginning of November, Mr. Schillinger required minimal assistance with functional transfers and supervision with bed mobilities. Mr. Schillinger was able to safely ambulate 40 feet with a rolling walker, ascend/descend 2 steps on the community stairs with handrails, and perform all ADL’s with minimal assistance.

Mr. Schilliner could always be found with a smile on his face. Mr. Schillinger continued to volunteer throughout his stay. He would regularly assist recreation with the daily newspaper delivery.

By the end of December, Mr. Schillinger was able to safely perform all ADL’s, ambulate 350 feet with a rolling walker, and could navigate 15 steps on the community stairs independently.

Prior to discharge, Mr. Schillinger shared, “Sans Souci has taken excellent care of me. I thank everyone who took care of me throughout my stay, especially the rehab) and nursing department. I would recommend Sans Souci to anyone in need of nursing or rehab care.”

Details of Experience:
Mr. Robert Walker arrived at Sans Souci Rehabilitation and Nursing Center on December 11th from an inpatient hospitalization at Westchester Medical Center. Mr. Walker was admitted to Sans Souci after sustaining a stroke and experienced left-sided weakness. Mr. Walker required a rolling walker for ambulation upon admission.

Mr. Walker was greeted by multiple professionals from the Sans Souci interdisciplinary team who had anticipated his arrival. Within the first 24 hours, he was greeted by clinicians, therapists, physicians, administrative staff, and many other valued members of the Sans Souci team to welcome him and answer any questions that he may have had.

Mr. Walker began working with his physical therapist, Sam, and occupational therapist, Mike, to improve ambulation and for completion of his daily activities. Mr. Walker began his rehab requiring moderate assistance for ambulation and ADL’s.
Mr. Walker shared he had a passion for music and enjoyed singing. He especially enjoyed the strolling violinist that frequently visits the Sans Souci community.

One week into his stay, Mr. Walker was able to ambulate 75 feet supervised using a rolling walker and were able to ascend 4 steps on the stairs.

Ms. Walker also enjoyed the luxury amenities at Sans Souci, such as the signature Hot Towel and Snack Cart program for residents.

Upon discharge, Mr. Walker was able to ambulate 300 feet using a rolling walker independently and ascend 8 steps on the stairs. Mr. Walker was safely able to perform bed mobilities and daily activities, such as grooming and dressing. Prior to leaving Sans Souci, Mr. Walker stated, “I had a great stay at Sans Souci.”

Details of Experience:
Ms. Martin arrived at Sans Souci Rehabilitation and Nursing Center on October 9, 2019, from Westchester Medical Center. Prior to coming to Sans Souci, Ms. Martin was involved in a motor vehicle accident that required her to have a total hip replacement. As a result, Ms. Martin had difficulty walking and muscle weakness. Upon completion of her hospital stay, Ms. Martin was referred to Sans Souci for physical rehabilitation to regain independence walking, navigating stairs, and performing activities of daily living (ADL’S).

Upon admission, Ms. Martin was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Ms. Martin to our community and conducted evaluations to custom tailor her stay.

Ms. Martin was evaluated by members of the rehabilitation team, Sam Ignacio DPT and Diane Gashi OTR/L, who performed an initial evaluation for her. Ms. Martin’s rehabilitation goals were to safely perform bed mobility tasks and functional transfers independently, ambulate 300 feet with a rolling walker, and safely ascend and descend 15 steps on the stairs. Upon evaluation, Ms. Martin was able to perform bed motilities and functional transfers requiring some assistance. Ms. Martin needed moderate assistance dressing and was able to ambulate 40 feet with a rolling walker, and was also unable to navigate the community stairs.

Ms. Martin appreciated the care she received from the facility staff during her stay. Ms. Martin shared that she felt comfortable and relaxed at Sans Souci due to the “homelike feel.” Ms. Martin also enjoyed participating in the daily recreation programs at Sans Souci.

Two weeks into her stay, Ms. Martin had made significant strides in her rehabilitation journey. Ms. Martin was able to complete her bed mobility and transfer tasks and ADL tasks, such as dressing, with supervision. Ms. Martin was walking 75 feet with a rolling walker and supervision and could navigate four stairs with minimal assistance!

Ms. Martin enjoyed nature and spent time observing the Sans Souci aviary in the mornings. Ms. Martin enjoyed the luxury amenities at Sans Souci like their signature Snack Cart Program provided by the community Concierge and daily newspaper delivery by the recreation team.

Ms. Martin continued to make significant strides throughout her stay. Three weeks into her stay, Ms. Martin was able to ambulate 150 feet with a rolling walker with stand by assistance, could ascend and descend 12 steps on the community stairs with moderate assistance, and could perform ADL’s and transfers with stand-by assistance.

At the week of discharge, Ms. Martin was able to perform bed mobility tasks and functional transfers independently. Ms. Martin was able to ambulate 300 feet, ascend and descend 15 steps, and perform ADL’s independently. At discharge, Ms. Martin shared that the rehabilitation and nursing care she received at Sans Souci was excellent and that she is grateful for the staff that helped her recover and return to life in the community!
