Details of Experience:
Mr. Collazo arrived at Sans Souci Rehabilitation and Nursing Center on April 4, 2019, from Montefiore Medical Center. Prior to coming to Sans Souci, Mr. Collazo had a high white blood cell count stemming from a wound to his lumbar back. Upon completion of his hospital stay, Mr. Collazo was referred to Sans Souci to begin receiving wound care and physical rehabilitation.

Upon admission, Mr. Collazo was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Mr. Collazo to our facility and conducted evaluations to custom tailor Mr. Collazo’s stay. Mr. Collazo required a wound vac to heal the skin wound he sustained prior to admission. At the initial evaluation by the wound nurse, Mr. Collazo’s wound was 13 cm.

Mr. Collazo began working with our Physical Therapist, Sam Beloni, and Occupational Therapist, Dexter Gallate, to begin working on bed mobilities, transfers, gait training, and performance of ADL’s. Mr. Collazo was bed-bound at the time of admission. Upon evaluation, Mr. Collazo was totally dependent on transfers, bed mobilities, and ADL’s.

At the beginning of June, Mr. Collazo’s wound had almost entirely healed. The wound was 1.5 cm and he no longer required a wound vac for healing.

By the middle of June, Mr. Collazo was able to perform bed mobilities and transfers with supervision. Mr. Collazo was able to ambulate with a wheelchair. At the end of June 2019, Mr. Collazo was able to perform ADL’s with minimal assistance.

During his stay, Mr. Callazo was an active participant in the daily recreation activities. He especially enjoyed attending recreation activities with Recreation Director, Leduc Le, and Recreation Therapist, Omar Garcia. Mr. Collazo is a very social individual and an avid sports fan, he enjoyed speaking with our Director of Concierge Services, Aryeh Schachter, and Maintenance Technician, William Dameus, about the latest news in sports.

Mr. Collazo enjoys the outdoors and prior to his admission worked in landscaping. Mr. Collazo enjoyed going out to the community patio and observing the trees, plants, and flowers on the grounds.

At the beginning of July 2019, Mr. Collazo was able to walk using Parallel Bars for 20 feet with moderate assistance. Within two weeks of using the Parallels Bars, Mr. Collazo began training with a rolling walker using assistance from two staff.

Upon discharge, Mr. Collazo had made significant progress in his physical rehabilitation. Mr. Collazo’s strength, endurance, and mobility had drastically since his arrival at Sans Souci. Mr. Collazo will be continuing his healing journey at an outpatient physical rehabilitation facility.
