Warren was admitted to Sans Souci Rehabilitation and Nursing Center from Saint Johns Riverside Hospital on September 20th 2016 secondary to new onset of decline in functional mobility. The patient had a medically complicated stay secondary to past medical history that was significant for vascular necrosis of left femoral head, arrhythmia, bacteremia, Charcot’s joint at ankle, Osteomyelitis of right ankle which occurred approximately two years ago and chronic left hip pain. When Warren was initially assessed by our therapy team, he presented with low self-efficacy requiring continuous positive reenforcement/encouragement for initiation and participation.
Additionally, Warren also demonstrated decreased retention and carryover during activities of daily living tasks and retraining. Despite the hardships he encountered along the way, Warren made steady and consistent progress throughout his stay at Sans Souci. He was able to improve baseline function from maximum assist and/or total dependence with attempts to initiate to modified independence/set-up. When Warren was ready to discharge on November 22nd 2016, he was able to ambulate up to 100 feet with rolling walker with supervision. Warren was ecstatic he made such a great recovery and is happy to be home with his loving family for the Holiday Season. This could not have been possible without the Amazing Therapy team at Sans Souci!!
