Details of Experience:

William is a 66yr old Male who was admitted from St Josephs’ Medical Center on March 11, 2016 with a diagnosis of CHF as well as hypoglycemia. Other Diagnosis includes COPD with continuous use of O2 via NC and cardiac arrhythmia. At time of admission William was confused with alters level of consciousness at times and difficulty following directions. He was at Max assist for bed mobility, transfers and was only able to ambulate 40ft with max assistance using RW. He was leaning forward and exhibited decreased stride length and toe clearance. Through much help from our Rehab and Nursing teams, William made great progress. When it came to the time of discharge, William was SBA with bed mobility activities and transfers.

He was able to ambulate 250ft with SBA and use of RW with improved toe clearance and stride length. He had some medication reduction which assisted in his ability to make self-understood decisions clearly and increased level of alertness. He continued on O2 but attempts to wean off were attempted. William was set-up assistance for toilet use, supervision for bathing, and supervision for Upper and Lower body dressings! William was so excited to go back to his wife and family and it would not have been possible if not for the wonderful team at Sans Souci!
