Details of Experience:

Patient was admitted to Sans Souci From St. Johns Riverside Hospital on April 19, 2015 with an Acute CVA. She was non ambulatory and required assistance with all ADL’s upon admission. During her stay she received a combination of physical, occupational and speech therapy to address the life changing challenges that came her way after her stroke. Upon her discharge home she was able to ambulate household distances, and perform ADL’s with supervision/ stand by assist. Patient was able to return to consuming the same consistency of food that she ate while at home before her stroke. Patient has made tremendous gains towards all her goals and we are proud to be part of her journey home.
