Details of Experience:
Doris is an 85 year old resident who was admitted from St. Johns Riverside on October 22, 2015 with a primary diagnosis of acute chronic respiratory failure with hypercapneia; She is Alert and oriented x2 to person and place. The Resident was deconditioned at that moment and would benefit from a restorative physical therapy program. Her functional status when she was admitted was maximal assistance for all ADL’s and oxygen dependent at all times. Doris was Unable to walk any distances alone. Doris, when she first arrived at Sans Souci was very weak most of the time.
As her Rehabilitation progressed, she garnered her strength to roll around in the hallways, socializing with other residents and staying pretty active. You could always find her sitting outside her room reading a book or talking with any passerby through the hallway. When Doris was discharged, her Functional activities vastly improved. She was able to do all her ADL’s with supervision. She did not need continuous oxygen as she needed when she first came to us. Most of all, she was able to ambulate up to 150 ft. with a rolling walker. A special thanks to our amazing Interdisciplinary team that made Doris feel like a new person when she was discharged home on January 22, 2016.
