Details of Experience:

Jeanette was admitted to Sans Souci Rehabilitation Center on January 8, 2016 from St. John’s Riverdale Hospital with Pneumonia and status post fall at home. When Jeanette was assessed by Therapy, she had exacerbation of increased need for assistance from others, reduced ADL participation, decrease in functional mobility, decrease in transfers, weakness, reduced functional activity tolerance and reduced static and dynamic balance. One of the goals of OT and PT was to increase functional activity tolerance and maximize the resident’s ADL’S. While Jeanette was ongoing therapy, another issue came to light. Jeanette hurt her left arm from her fall at home and required total assistance for her upper extremities.

It came to a point where Jeanette did not know which direction to go in as surgery or therapy were her options. After meeting with our Professional team, Jeanette felt most comfortable with therapy as it had the best prospective results. After aggressive workouts, Jeanette started to make great progress as she was almost ready for discharge. Upon discharge from Sans Souci, Jeanette was able to perform basic ADL’s with minimal assistance and she was able to ambulate up to 75 ft. with a rolling walker. On March 24, 2016 Jeanette left our facility elated, accompanied by her spouse as she planned on continuing to get better and stronger at home.
