Respite Care:
We understand that caring for a loved one who is ill can be exhausting and emotionally draining. When caretakers need to rejuvenate or travel, we provide respite services. We will care for your loved one overnight or for an extended stay.
Social Services:
Upon admission, Sans Souci social workers play a vital role in improving the quality of our patients’ lives, because it is more than good physical care that helps a patient recover from accident, surgery or illness. Our social workers address the emotions and stress patients and their families often experience, by helping patients acclimate to their new surroundings and daily routine.
Our social workers advocate for residents, provide daily support and watch for signs of depression or other serious mental conditions. They connect patients and families with resources in and outside the nursing home and facilitate transitions such as the return home, a hospital stay or hospice. They help guide families, patients and care providers through difficult conversations or conflicts and help them cope with illness, emergencies, and long-term disease. Moreover, our social workers educate patients in healthy living when they are ready to return to the community and begin the discharge planning process as soon as the patient is admitted. The transition back home is facilitated as our social work staff make all arrangements such as home care and medical equipment prior to discharge.
Hospice care:
Designed to provide comfort and support for terminally ill patients who have exhausted all treatments and procedures and have limited life expectancy, hospice care at Sans Souci provides comfort care and support to families. Of paramount importance is the patient’s dignity, pain control and symptom management.
Subacute Care
Sans Souci is recognized as a leader in providing quality, sub-acute medical care.
At Sans Souci, our subacute care unit, merges the sophisticated technology of a hospital with the efficient operation of a skilled nursing facility. Our subacute patients are recovering from an acute illness, injury, or cardiac event and benefit from the hi-tech care in our subacute unit. They receive highly-skilled care from doctors, therapists, nurses and specialists who work in concert with the patient and family members to formulate a goal-oriented plan of care for each resident. The Subacute care team is dedicated to providing appropriate interventions to avoid unnecessary and frequent hospitalizations and focuses on training patients to achieve their highest level of independence as quickly as possible.
This goal oriented treatment after hospitalization helps our patients return to their optimum level of activity and physical well- being before returning home.
When looking for subacute care ask the facility these questions:
· Does the facility offer the therapies and medical services necessary for the rehab program?
· Does the facility have up to date rehab equipment?
· Is the staff trained to work with rehab and subacute facilities?
· Does the facility have a separate dedicated subacute unit?
